Support for adults with disabilities
Practical support built around how someone wants to live their life.

What this includes
- Support with daily living, on the person's own terms
- Help with work, study, volunteering and social life
- Personal care where it is wanted
- Support to use direct payments and personal budgets
- Working alongside family and other professionals
Who this suits
Adults with a physical or learning disability living in their own home.
How it is planned and reviewed
Our Registered Manager assesses at home before anything starts, and writes the care plan with you. It sets out the visits, the times, the tasks and the cost, and you keep a copy.
We check in during the first week and review formally within six weeks. You do not have to wait for a review to change something — tell us and we will change it.
