Palliative and end of life care

Comfort, dignity and support at home in the last months or weeks.

A carer helping an older person with medication at their bedside.

What this includes

  • Comfort, personal care and help with symptoms
  • Working with district nurses, GPs and hospice teams
  • Support for the family as well as the person
  • Respecting known wishes and advance care plans
  • Continuity, so familiar faces are there at the end

Who this suits

People who want to stay at home, and the families supporting them.

How it is planned and reviewed

Our Registered Manager assesses at home before anything starts, and writes the care plan with you. It sets out the visits, the times, the tasks and the cost, and you keep a copy.

We check in during the first week and review formally within six weeks. You do not have to wait for a review to change something — tell us and we will change it.
